Below are links to the forms that can be filled out for the vaious purposes described.

 

Activity Reporting Form is to be used by Directors (Church, Community, Council, Youth, Family and Program) as well as committee chairmen to report their hours to the Council's Program Director.  This automatically gets filled in a spreadsheet and helps the Grand Knight and Program Director report the activities down to Indiana State council.

 

 

Volunteer Form.  This form lists all current and recent past Council Activities.  Please take a moment to fill out this form.  This will enable us to gage who is interested in helping out for what activities.  Showing an interest in an activity does not commit you to volunteer your time.  It will allow the various organizers for the different activities to see what volunteer resources can be available.  This will allow for smoother transitions to take place from month-to-month or year-to-year as different people may take on responsibilities for organizing an event. 

 

 

Register Email Form.  Use this form to register your email with us.  This will allow us to eliminate the paper newsletter from being sent to every Knight who has an email account or web access - benefitting the environment and enabling us to keep more of the Council's funds dedicated for charitable causes.
